HOLD HARMLESS AGREEMENT, ASSUMPTION OF RISK AND LIABILITY RELEASE

In exchange for shoes and services, the undersigned agrees to hold Shoes for Your Soul, a non-profit organization, harmless and assumes all liability, risks, claims, and demands whatsoever that may hereafter accrue against Shoes for Your Soul and any other person, corporation, association, or partnership charged with responsibility for injuries to the undersigned’s person or property, the treatment thereof, and all resulting consequences as a result of said services and any resulting damages.

Furthermore, the undersigned acknowledges receipt of valuable shoes, socks, and services. These services are accepted as sole consideration for the final release and discharge of all actions, rights, causes of action, claims, and demands whatsoever that now exist or may hereafter accrue against Shoes for Your Soul and any other person, corporation, association, or partnership charged or who may be charged with responsibility for injuries to the person or property of the undersigned, the treatment thereof, and all consequences flowing therefrom as a result of said services.

The undersigned warrants that:

1. No promise or inducement has been offered beyond what is stated in this agreement.
2. This Hold Harmless Agreement, Assumption of Risk, and Release is executed without reliance upon any statement or representation by the persons or parties released, their representatives, or physicians concerning the nature or extent of injuries, damages, or legal liability.
3. The undersigned is of legal age, competent to execute this release, and accepts full responsibility.

The undersigned further agrees that this agreement applies to all known, unknown, and anticipated injuries or damages resulting from the said accident, casualty, or event.
